Making a Claim
  
  If your policy started or renewed on or prior to 30 November 2011, your underwriter is Allianz Worldwide Care until your next renewal date (tip: your policy number will commence: P000).

To avoid any delays in settling your claim, please ensure that you send your completed claim form and any supporting documents (paid invoices, medical reports, etc) to the address shown below.   
 
 When you need to make a claim, we want to make sure that the process is kept as simple and stress free as possible for you. So before you see your doctor or start any treatment, please read the section in your policy documents under the heading 'Assistance & Claims Procedure' carefully, as this will help ensure that your claim is settled quickly.
 

In the event of a planned admission to a hospital, it is important that you obtain pre-authorisation from us prior to your admission, as failure to do so could result in your claim being declined. If your treatment is subsequently proven to be covered under the terms and conditions of your policy, we will only pay 50% of the eligible benefits.

Claim forms & what to do next

Copies of our claim forms may be downloaded using the links to the left, but should you experience any difficulties, please contact your nearest ALC Health office and we will arrange for copies to be forwarded to you.

All claims should be submitted to us, within six months of your treatment taking place, with a fully completed claim form, original invoices and receipts together with any supporting documents about your claim.

Before you make a claim, it is important that you familiarise yourself with the terms of your policy and the cover it provides, as there may be certain restrictions that have been applied to your cover.
