Routine & Restorative Dental Treatment
Where you have chosen ALC Health's optional dental benefit.

Dental treatment and all services performed in a dental surgery or clinic which do not require your admission as an in-patient are often referred to as out-patient treatment and should generally be paid by you at the time treatment takes place and then submitted along with a completed claim form to us for reimbursement.

As you will need to complete and submit a “dental claim form” to enable you to reclaim those eligible costs that you incur under your policy, please take a claim form with you on your first visit to your dental practitioner or surgeon.

Important : If we do not receive a fully completed claim form or any supporting documents are not provided, there may be a delay in settling your claim as we will need to revert to you or your medical provider for any information that might be missing.
