Routine & Restorative Dental Treatment
Where you have chosen ALC Health's optional dental benefit.

Dental treatment and all services performed in a dental surgery or clinic that do not require admission as an in-patient are often referred to as out-patient treatment. This treatment should generally be paid for by you, with you then submitting the receipts along with a completed claim form to us for reimbursement.

As you will need to complete and submit a Dental Claim Form to reclaim eligible costs that you incur under your policy, please take a claim form with you on your first visit to your dental practitioner or surgeon.

Important : If we do not receive a fully completed claim form or if any supporting documents are not provided (don't forget to keep copies for yourself), there may be a delay in settling your claim. This is because we will need to revert to you or your medical provider for any information that might be missing.
